Patient no-shows: how to measure the loss and how to stop it

How many patients missed their appointment last month? If you do not know, you are not alone. Here is how to count it in ten minutes, turn it into money, and what actually stops it — even without buying anything.

11–30%no-show rates recorded by earlier studies in Saudi Arabia
21%measured by a 2025 study in a Riyadh hospital's outpatient clinics
100appointments lost a month by a clinic seeing 400, at 25%
0published studies we found for Iraq — which is why your own number matters most

First: the number

The number nobody knows

A paper diary records who booked, never who failed to arrive, so the information is gone when the day ends. Calculate it by hand this week, in ten minutes — it is usually worse than the guess.

  1. 1

    Take last week's diary

And set aside the cancellations: a patient who cancelled told you, and did not go missing.

  2. 2

Count who came, and who did not

How many patients attended, and how many did not show without telling you.

  3. 3

Divide the no-shows by the total

No-shows ÷ (attended + no-shows). That is your rate — the same way Mawid's dashboard calculates it.

Second: the money

Turn the rate into a sum

Every missed appointment is capacity you have already paid for: the rent is paid, the staff are paid, and the chair is there. Put in your clinic's numbers.

You will not recover all of it. Recovering a fifth is enough.Missed appointments × average appointment value = monthly loss400 × 25% = 100 · 100 × 50 = 5,000
Third: the causes

Why patients miss appointments

Forgetting

By a wide margin the leading cause, especially for visits booked a week or more ahead.

No easy way to cancel

A patient who knows they cannot come and has no simple way to say so stays silent, and the slot stays blocked until it expires.

Never really confirmed

Booked in a long WhatsApp thread nobody closed, with no one checking the patient read the last message.

Genuine circumstances

They cannot be prevented, but they can be known early enough to refill the slot.

Most of the first three are solved by a reminder and an easy reply.

Fourth: the fixes

What works, and what does not

What actually works

  • The 24-hour reminderHandles forgetting and leaves enough time to refill if the patient cannot come.
  • The 2-hour reminderCatches the rest.
  • A one-digit replyMakes cancelling easier than silence. That is deliberate: an early cancellation is an opportunity, not a loss.
  • An instant alert to receptionTurns that cancellation into a different appointment the same day.
  • A standing list of patients who want an earlier timeMakes refilling one phone call. Today that is a list at the front desk, not a screen in the system.
  • A monthly measurementBecause what is not measured does not improve, and because you need the number to know whether any of this is worth it.

What does not work

  • Calling every patient by handIt works, until the first busy day. Reception has other work.
  • SMSIt arrives, but it is not read the way WhatsApp is read, and it cannot easily be replied to.
  • Financial penalties for no-showsThey create arguments with patients, get waived anyway, and cost more reputation than they collect.
  • OverbookingIt turns an empty-chair problem into a crowded-waiting-room problem, and angry patients.
Fifth: the test

Start next week, without buying anything

We say so because we would rather work with a clinic that knows its number than one that buys without knowing it.

  1. This week

    Calculate your rate

For last week, with the three steps above.

  2. Next week

    Remind every patient by hand

Reception sends a reminder the day before every appointment, for one full week.

  3. Straight after

    Calculate the rate again

The same way, over the week of reminders.

The gap between the two numbers

Is the size of the problem a system can take off your hands. If it is small, buy nothing. If it is large, you have just found a monthly sum you have been losing.

With Mawid

Step two, every day, without anyone remembering

Mawid sends the reminder a day and two hours before every appointment, and the patient replies with one digit. If they cancel, reception knows at once and can fill the time. At the end of the month, the rate is calculated the same way you calculated it by hand.
